If you’re a physician, therapist or other service provider looking for information about services for kids and youth with disabilities and developmental needs or want to make a referral, this is the place for you. CTN’s focus is on supporting kids and youth who need long-term support for a range of diagnoses, disabilities and developmental needs from birth to their 19th birthday (or up until high school graduation). Our clients often require multiple interventions such as physiotherapy, medical, speech and/or occupational therapies. Please note that some of our clinics, programs and services stop serving kids and youth at specific ages due to ministry guidelines. If you have a question about when specific services will end for a child please contact us.
CTN provides service navigation and referrals to various programs and services throughout the Simcoe County and York Region. If you think a family you work with needs to access any of CTN’s services, all you need to do is make the referral and we will do the rest. For School Based Rehabilitation Services (SBRS) occupational therapy, physiotherapy and speech language pathology, school board staff must submit referrals via the School Portal. Click here for all referral form information.
To discuss consent to share information with clients and families please review the Consent and Privacy Guide with the family and complete the Consent for Information Sharing and Collection of Personal Information form in the guide. This will facilitate the sharing of information and the use of the shared record across all providers working with the child, youth and family.
All completed forms can be faxed to CTN ACCESS at 705-792-2775.
