The Neuromotor Clinic offers consultation services for kids and youth with neurodevelopmental conditions that affect muscle tone, such as:

cerebral palsy
significant low tone (hypotonia)
high tone (hypertonia)

The needs of kids and youth can be complex and change over time. The clinic provides a comprehensive physical examination and consultation that includes a discussion of:

medical history
current physical functioning
community and school supports/services

In some cases, consultation may also be provided to confirm a diagnosis.

What We Do

The Neuromotor Clinic team:

Provides interdisciplinary diagnostic assessments.
Provides neuromotor and medical consultation to kids and youth with complex medical and developmental needs.
Monitors needs over time and collaborates with other clinics and community-based service providers as appropriate.

Eligibility

Neuromotor Clinic services are available to kids and youth:

Up until the age of 18
With neuromotor diagnosis or similar presentation
With cerebral palsy and a Gross Motor Function Classification Score of:
- GMFCS of 3, 4 or 5
- GMFCS 1, 2 (walking) if under four years old with complicated medical conditions and primary physical concerns
With neuromotor concerns discharged from Neonatal Follow-Up Clinics (until transition to school)
Who were referred from another Children’s Treatment Centre by a specialist
Who require consultation for confirmation of neuromotor diagnosis
Who live in York Region

Referral

Kids and youth must be referred by their physician. Complete the online Physician Referral form, or to send your referral via fax, use this form.
