About event:

The meetings are 60 to 90 minutes in length with case managers who can assist families with:

• Application forms for ODSP, SSAH, Mobility transit
• Navigating and understanding the developmental services system
• Locating and accessing appropriate resources
• Information and resources for transition to adult services

Resource Days—One-on-One Meetings:

Mon., Mar. 5* - 9:00 a.m. - 4:00 p.m.
Thurs., Mar. 8 - 9:00 a.m. - 4:00 p.m.
Thurs., Apr. 19 - 9:00 a.m. - 4:00 p.m.
Tues., May 15 - 9:00 a.m. - 4:00 p.m.
Mon., June 11* - 9:00 a.m. - 4:00 p.m.
Thurs., June 14 - 9:00 a.m. - 4:00 p.m.

* Transition Planning Case Manager available onsite to help with transition planning.

Appointments are made on a first-come, first-served basis.
