Who should attend?    


A 60-90 minute face-to-face meeting with a case manager who can assist families with:

  • Application forms for ODSP, SSAH, Mobility transit
  • Navigating and understanding the developmental services system
  • Locating and accessing appropriate resources
  • Information and resources for transition to adult services
